Job Summary:

The Home Health Scheduling Coordinator is responsible for managing the daily scheduling needs of patients and caregivers. This position ensures patient care is staffed according to clinical needs and insurance authorizations while maintaining clear, compassionate communication with families and employees. The Scheduling Coordinator works closely with multiple departments to support timely, high-quality home health services and create a positive experience for Tendercare patients and staff.

Primary Responsibilities:

• Coordinate and maintain caregiver schedules in alignment with patient needs and insurance-approved hours.
• Serve as the main contact for scheduling issues, including call-offs, hospitalizations, and shift coverage changes.
• Communicate with patients, caregivers, and internal teams via phone, text, email, and internal systems.
• Monitor and clear scheduling alerts in the electronic medical record (EMR) system.
• Collaborate with other departments to support consistent, high-quality care delivery.
• Document schedule-related updates accurately and in a timely manner.
• Ensure HIPAA compliance and maintain patient confidentiality in all communications.
• Provide additional administrative and scheduling support as assigned by leadership.
• Participate in on-call or after-hours scheduling coverage as needed.
