About the Role

We are seeking a Board-Certified Primary Care Physician to provide comprehensive medical care to Veterans residing in a VA Domiciliary (DOM) / Mental Health Residential Rehabilitation Treatment Program (MH RRTP) at the Bath VA Medical Center in Bath, NY. The physician will work within an interdisciplinary VA healthcare team to deliver patient-centered, evidence-based primary care to a geriatric and medically complex Veteran population, in compliance with VA standards and federal contracting requirements.

Responsibilities

• Provide full-spectrum primary care services to Veterans residing in a VA Domiciliary (DOM) setting, including preventive care, chronic disease management, and acute medical evaluations.
• Manage medical conditions commonly seen in geriatric and residential rehabilitation populations, including chronic illness, mental health comorbidities, substance use disorders, and functional limitations.
• Conduct physical examinations, diagnose and treat conditions, and order and interpret appropriate diagnostic tests.
• Collaborate closely with mental health providers, nursing staff, social workers, addiction specialists, and other interdisciplinary team members.
• Participate in care planning meetings, goals-of-care discussions, and continuity-of-care initiatives.
• Document all patient encounters in the VA’s Electronic Health Record (EHR) in accordance with VA and federal documentation requirements.

Qualifications

• Board Certification in Primary Care, Family Medicine, or Internal Medicine.
• Unrestricted medical license in any U.S. state or territory.
• Active DEA registration with the ability to prescribe medications.
• Ability to meet VA credentialing, background investigation, and security clearance requirements.
• Experience with electronic medical records (EMR/EHR).

Required Skills

• Prior experience working in a VA Domiciliary (DOM), MH RRTP, nursing home, or geriatric care setting.
• Familiarity with Veteran populations and VA healthcare systems.
• Experience managing patients with co-occurring medical, mental health, and substance use disorders.
• Strong interdisciplinary collaboration and patient-centered communication skills.
