As a Behavioral Health Therapist at Catalytic Health Partners, you can play a crucial role in helping patients with behavioral health needs get the care they desire and need. You will work giving therapy to patients while we get their services established at a behavioral health home. You will advocate for their care in behavioral health facilities and other settings ensuring their desires and needs are met.

What We'll Expect from You and What You'll be Doing:
·Spending your day seeing 5 members per day, in the member’s home, engaging with them on their behavioral health needs, as well as their whole health needs.
· Working on an integrated team to best ensure all needs are being met and addressed.
· Clearly explaining the benefits of our program and how it caters to their unique healthcare needs.
·Gaining patient trust and consent for the program, ensuring they feel cared for and understood.
·Arranging the patient's intake to behavioral health clinics as well as providing support
for the best care from their clinics.
·Regularly updating medical records and coordinating with the healthcare team for seamless
patient care.
·Taking on additional responsibilities as required for effective patient support.
Location:
· East Maricopa (Gilbert, Mesa, Apache Junction, Chandler)
Requirements:
·Legally authorized to work in the US.
·Be registered with the Arizona Board of Behavioral Health Examiners with an active
license.
·One year or more of clinical practice is preferred (but not required).
·Strong skills in organization, time management, and adaptability to change.
·Excellent communication skills, both in explaining complex information and in actively
listening to patient needs.
·Empathy and teamwork skills are highly valued.
